Disclaimer * The Community Partner Network is stewarded by the Transformation Exchange, LLC. The Transformation Exchange LLC seeks to equip the community to serve in the gap by developing an inventory of faith-based and community services that are focused on helping people in need. The information collected will help people find community resources and help connect these resources to non-profits, service providers, and civic organizations increasing their capacity to serve marginalized community members in Wake County. Participants are provided with the opportunity to keep their survey private by choosing not to have it included in the featured Tool. The Transformation Exchange will assist in connecting the participant with service providers/non-profits that share like passions and mission fields instead. The Service Provider would then be expected to contact the participant to initiate a community partnership. The information displayed in the Tool originates from public data bases such as Census and Geographical Information Systems (GIS). The Transformation Exchange strives to facilitate access to this vital information to help community service providers be more effective, save time, and preserve their resources. Participating in the survey is voluntary and not required to use the Tool. Participants choosing not to have their survey information added to the Tool, but desiring connectivity with a service provider, will still be helping to increase the capacity of our service providers to stand in the gap. Only the service provider(s) will be notified of the available community resources and the only addition to existing public data provided in the Tool will be a Community Partner notation. Participant information will not be sold, misused, or shared with public or private entities. Its use will remain limited to its intended purpose in the Transformation Exchange Community Resource Tool.
